Application Forms:

Veteran Application

VETERAN APPLICATION: The North Country Honor Flight recognizes the service and sacrifices of our World War II Veterans by offering to you, a Free all expenses paid trip to Washington D.C. to visit your National World War II Memorial dedicated in your Honor.   We are currently accepting ALL veterans from WWII and/or terminally ill veterans from all wars. In the future, North Country Honor Flight will expand to include veterans from the Korean and Vietnam wars.

Guardian Application

Guardians are essential to the success of the Honor Flight Program. Without these volunteers we would not be able to carry out the dreams of so many veterans, enabling them to see the memorials built in their honor.   From the moment the veterans are in our hands until we return them to their homes in the evening, Guardians will treat these heroes as if they are family and ensure that every veteran has a safe, memorable, and rewarding experience. Guardians are volunteers and are expected to pay for their trip.

Volunteer Application

If you would like to volunteer, we can always use your help. You may also contact us by phone or eMail to volunteer. Please call us with any questions (518) 834-9901.

Several of our officers or Board Members will visit each WW-II veteran who is interested in signing up for an Honor Flight. At this time we bring the forms for both the Veteran and Guardians, and assist in filling out each form. Information that we request includes service records and details of all medications and prescriptions. It is very helpful if you can have this information prepared in advance of our visit. ALL WW-II veterans qualify for the Honor Flight with no conditions or restrictions!
